I enter into this program with the full understanding that I am responsible for creating my own results by fully participating during each appointment and doing the set homework after each session.

I acknowledge that Chalisa does not work in the areas of domestic violence, clinical psychology or addictions.

I understand that rescheduling or cancelling of an appointment needs to  be done at least 24 hours before my scheduled appointment and that appointment changes within 24 hours of my scheduled start time incur a cancellation fee.

I understand that I can request to stop my program at any time and request a refund of unused hours. A refund of unused hours will be given minus all fees associated with the initial purchase (credit card / paypal fees) + administration cancellation fee of $75.
